Re: 'Surnames' of characters
There's also at least Jirouta Ishibuchi from GDWO, Reiji Hidaka from the Gaiden series, and of course the Kurohabaki family. Benimaru from GDA also refers to himself as Benimaru Itou once before his death.
I really wish we got some canon explanation for why some dogs take up surnames. Going by Benimaru's case, it's plausible that dogs who belong or used to belong to humans took up their master's surname, but then you have characters like Kyoushiro (yes, I believe Shirogane is meant to be a surname and it being translated like a title into "the Silver" is a translation error) who apparently just gave themselves surnames at random.
As for clear titles, rather than surnames, I can think of:
Ben: Ouu no Fudou Myou-ou (Fudō Myō-ō of Ohu), Honoo no Shoukou (Blazing Officer), Toushou (literally Brave Leader, translated as Warmaster in the Finnish editions)
Mukonga: Tetsujin (Ironman), Rekkabou (Wildfire Monk)
Kite: Fuujin
Batto: Shingan (Mind's Eye)
Kai Bros: Kai no Maken (literally Demon Dogs of Kai, translated as Hellhounds of Kai in the Finnish editions)
Kurotora: Hebigoroshi (Snakeslayer)
Fuuga: Ougon Senshi (Golden Warrior)
Akakabuto: Kyouyuu (Mad Bear)
Moss: Kasumidake no Enma Daiou (Yama of Kasumidake)
Suiga: Sanzu no Kawa no Banpei (Guardian of Sanzu River)
Matheus: Shiroi Urufu (White Wolf)
George: Hagure no George (George the Loner)

Re: 'Surnames' of characters
Aside from his introduction, was Kyoushirou ever called Shirogane or Shiro (according to wiki) again?

Re: 'Surnames' of characters
John the Eastern Tiger
also, Gin's surname is said to be Ohu. Gin from Ohu, not Nagareboshi. That would be title, not surname..

Re: 'Surnames' of characters
John is added under Titles. ^^
Also, there's a reason Gin Nagareboshi is both under Surname and Title, as it kinda fits both. Specially if going after Finnish translation that translated it to "Gin Tähdenlento" as in "Gin Shooting Star" where Shooting Star/Nagareboshi is treated as a surname.
And Gin from Ôu is describing where he's from. Like "Akatora form Kai", "Ben from Kofu" "Akame from Iga", etc. It's not a surname. Not more the me would be Tora from Sweden (or the city/area in Sweden I would descibe myself to be from). ^^

Re: 'Surnames' of characters
Thanks for explaining, and here's some more I remembered:
Hakuro the Brown Bear Killer
Akakabuto the monster
Matheus was also called White Demon
Also, I thought it could be interesting to create a list of their jobs/occupations as well, such as assassins, generals, leaders, vassals, trainers, scouts, messengers, etc?
EDIT:
Ogasawara Gennai, according to pg135 vol 14 GDWO

Re: 'Surnames' of characters
Hakuro was called the 'Demon King' in the Russian arc, when he died.
There's that silly dog who said he gets called the 'snapping turtle' in the anime, during Kurotora's nephews' fight lol

Re: 'Surnames' of characters
The narration specifically calls Hakurou the "Demon King of Douhoku" (道北の魔王) immediately before and after his death.
The dog you're thinking of is "Pepe the Snapping Turtle"... though watching the scene, it sounds like he calls himself "suppon no Pepe" (スッポンのペペ), and "suppon" refers to a completely different species of turtle (either the Chinese soft-shelled turtle or soft-shelled turtles in general). The Japanese word for the (common) snapping turtle is "kamitsukigame" (カミツキガメ) instead.
That said, "Snapping Turtle" as a title is probably a perfectly acceptable localization because "Pepe the Soft-Shelled Turtle" doesn't sound quite as intimidating in English

Re: 'Surnames' of characters
This chapter image from Shounen Jump features some old titles and some I don't think have been mentioned before.
Gin: Shooting Star (流れ星 nagareboshi)
Akame: Iga Ninja Dog (伊賀忍犬 Iga ninken)
Kurotora and Chutora: Demon Dog of Kai (甲斐の魔犬 Kai no maken)
Ben: Blazing Officer (炎の将校 honoo no shoukou)
John: Sniper (written with 狙撃手, the kanji for "sharpshooter", but pronounced as スナイパー/sunaipaa)
Musashi: Yokozuna of Tosa Fighting Dogs (土佐闘犬横綱 Tosa touken yokozuna)
Cross: Ben's Wife (ベンの妻 Ben no tsuma)
Moss: Devil of Kasumidake (霞岳の魔王 Kasumidake no maou)
Fuuga: Golden Wolf of Legend (伝説の金狼 densetsu no kinrou)
Retsuga: Hakkenshi (八犬士)
Noroi: Commander of the Hakkenshi (八犬士の総帥 Hakkenshi no sousui)
Reima: Boss of the Wolves (狼族のボス ookamizoku no bosu)
Hyouma: Young Noble of the Wolves (狼族の若 ookamizoku no waka)
John is pretty interesting since his title is... y'know, the name of a whole different character. And then there's poor Cross, whose greatest achievement in life apparently is being married.
